General
Please note:
1. Verification Sheets will be sent out on the first day of school and will need to be returned by Friday, September 9th, 2011.
2. Ongoing Excursion Forms will need to be filled out and signed by every student by Friday, September 9th 2011 Ongoing Excursion Form.pdf
3. Students who will be turning 18 during the 2011-2012 school year or who are already 18 will be required to fill in the "Permission to Contact Parents form" by Friday, September 9th 2011 PermissiontoContactParents.pdf
4. Acceptable Use Agreement Forms will neet to be filled out and signed by both students and parents. Please return the form to school by Friday, September 9th 2011. AcceptableUseAgreement.pdf
5. Consent Form - Grant /not grant permission for the school board to display student work, special awards, pictures in yearbook, website etc. Consent Form (post picture etc).pdf
Medical Forms
(Only for Student with Medical Conditions) - Students who picked up their report cards would have received a medical form with a return stamped envelope to return their medical forms over the summer. All other student will be receiving their forms in their MSIP classes and will need to return the forms ASAP (no later than Friday, September 9th, 2011)

Anaphyalxis Forms (required by all students with a life threatening anaphylatic condition) Have been picked up already by students who have picked up their report cards over the summer or will be sent home the first day of school.
