Coop Student Information
The following provides students with important information/documents related to the cooperative education program. Please check back on a regular basis for updated information.You can also visit our external website for more information and access to assignments.
Pre-placement Assignments:
Pre-placement tracking- Provides students with a list of all assignments, forms and tests that must be completed prior to starting their placement.
Pre-placement Forms:
** All students who are under the age of 18, must have a parent/guardian signature on each form.**

Log Sheets
Log Sheet En.doc Log Sheet Fr.doc- print off log sheets and use them to record hours worked at the placement. Remember log sheets are due every Tuesday. All day coop students can fax log sheet to CCVS (613-932-8128). Please ensure the fax includes a cover sheet that indicates the Coop teachers name.
WEA Amendment Form.doc - if you plan on working outside of regular coop hours (8am-5pm) you must fill out a WEA amendment form. The form must be completed and signed by your coop teacher PRIOR to completing the extra hours.
